A 22 year old female presents with fever in the 2nd trimester of her 1st pregnancy she gives history of dysuria and loin pain. On examination she is febrile and has bilateral tenderness in the loins. She gives a history of recurrent urinary tract infections in childhood. There is a family history of hypertension and she has been told that her mother has kidney disease.

Urine examination shows pyuria and culture of urine demonstrates a significant growth of E. coli.
