A 47 year old female presents with a history of breathlessness, constipation and abdominal swelling. There is no significant past medical history and she is not on any medication.

On examination you note that the patient looks thin, pulse 90 beats per minute regular, JVP elevated and the JVP is increased during inspiration. There is ascites. A chest X-ray is performed and this shows pericardial calcification.

In this patient what is the likely underlying diagnosis?
