A 55-year-old Caucasian male
presents with a 2-year history of arthritis, fever, recurrent cough and chest
pain. He has been feeling generally unwell. Recently he has developed diarrhoea
(steatorrhoea), abdominal pain and weight loss.
On examination he is pigmented,
there is finger clubbing and lymphadenopathy. On auscultation of the heart a
pan-systolic murmur is heard.
